Grant Overview

The 2025 Connected Care Innovation Grant is an initiative from the Infoway Centre for Clinical Innovation in Digital Health (CIDH), which supports national efforts to accelerate the adoption of healthcare interoperability. Through the CIDH, Infoway continues to drive initiatives that encourage engagement with clinical leaders and learners, supporting the adoption of Connected Care solutions.

Eligibility

Eligible Applicants*
  • Clinicians, including physicians, nurses, pharmacists, and other allied health professionals
  • Health provider groups, including specialist clinics and primary care clinics (solo practitioner clinics, PCNs, FMGs/GMFs, FHTs, FHGs, FHOs, CHCs, AHACs, nurse practitioner-led clinics, etc.)
  • Health organizations and providers, including public health units, mental health service providers, OHTs, outpatient specialist clinics, home and community care, long term care, etc.

* While these are the listed eligible applicants, we will consider all reasonable community-focused proposals.

Eligible Projects

Eligible projects should ideally align with one or more of Infoway’s strategic goals as outlined in the interoperability roadmap, aim to complete the project within six months, demonstrate clear skills and experience in delivering projects of this nature and support activities within Canada. Criteria

The proposed initiative should:

  • Align to at least one of the pillars in Infoway’s Shared Pan-Canadian Interoperability Roadmap
  • Solve a common pain point for clinicians, ideally reducing their administrative burden
  • Leverage digital health technologies in a novel manner to improve Connected Care in Canada
  • Have the potential for scalability, replicability, and sustainability
  • Be capable of being implemented quickly so it can provide immediate benefits to providers and patients, ideally in six to nine months
  • Identify a measurable improvement that has a clear target for success
  • Enhance the quality of data captured and/or exchanged within the circle of care

The proposed initiative could also:

  • Involve an interdisciplinary clinician team
  • Involve a patient partner or significant patient voice
  • Demonstrate positive impact on rural and remote, Indigenous and/or marginalized communities
  • Involve leveraging AI-powered technologies as a catalyst to improve the experience of clinicians and patients, with a particular focus on primary or community care settings.
