This evaluation of a project supporting patients with COPD in Vancouver Coastal Health demonstrates the effects of home health monitoring. Patients reported a 90% satisfaction rate with the program. Hospital administrative data indicated a 47% decrease in overall ED visits, an 88% decrease in hospital admissions, and a 50% decrease in the median length of stay.
