* If you are registering as a student, please send proof of current student status (i.e. scan a copy of student ID or proof of registration in the current academic year). Please include your full name and the name of the event, "2017 Infoway Partnership Conference” in the e-mail.

Payment, refund and cancellation policies
Advance registrations received with payment by October 13, 2017 will be given the early registration discount. Registrations after that date will be charged the regular fees.
All payments must be received in full before the event. Outstanding balances will need to be paid on-site prior to admission to the event.
Delegates may elect to transfer their registration to another individual. This change must be requested via email.
Refunds will be issued upon request, less a $100 administration fee (plus applicable HST), if notification is received via email by October 13, 2017. No refunds will be made after this date.
Canada Health Infoway reserves the right to cancel should a minimum registration number not be attained.
