Digital health systems need to interact with each other across all care settings so that Canadians’ health information moves with them through the system.

Since a patient’s care journey may include check ins with a family doctor, visits with specialists, trips to the emergency department, and more, everyone on a patient’s care team needs better access to a patient’s complete health care history so delays and duplications can be avoided.

Connected care, or interoperability, enables information to flow seamlessly between different solutions and devices. When different parts of the health system are interoperable with each other, they can speak the same language.

During this year's Digital Health Week, we want you to help us get the word out about the importance of a modernized health care system that is connected, equitable and accessilble.
