This session will share lessons learned from a multi-incident analysis conducted by ISMP Canada on an eight-month provincial pilot project that will be relevant and applicable to community pharmacy practitioners in other provinces of Canada. It is designed for pharmacists and pharmacy technicians who would like to have an understanding of how continuous quality improvement can be achieved through medication incident reporting and analysis. Safety recommendations will be presented based on the medication workflow process.
